Why should I buy a card?

Give the gift of a wellness exam (check-up) or other medical procedure to a loved one who may not have the financial means to pay for it. Show that you care enough to give the gift of life.

How does the card work?

Click the Buy a Card link to purchase your gift card. The card will be sent by email to you or the recipient (your choice). The recipient activates the card online by following a link in the email or by calling our office. We email the recipient to notify them of the closest facility where they can redeem the card. The recipient prints out the card and makes an appointment. If the recipient does not have internet access, the card and facility information will be mailed to them. Life Gift Cards^™ are not reloadable, are for one-time use and may not be redeemed for cash. The card does not have an expiration date.

What services can the cards be used for?

The cards can only be used for a wellness exam or a mammogram screening procedure at this time. Other related services and fees are not covered and are the responsibility of the card recipient.
